Once my pain started to increase the doctor put me on a narcotics patch, which made me a little dizzy at first but my body adjusted to it. He started with 12 mg and then doubled it to 2 patches almost right away. Sometimes I have to augment with Ibuprofen and definitely take Ibuprofen PM at bedtime.
