I had fat grafting but it didn't make any difference. I had radiation so the skin was so tight the fat couldn't "poof" out the skin.

I had radiation in this breast too, so that's another concern. But my skin isn't tight; it looks like there is space for the fat to reside. I also read that the fat can die and turn blue/black - necrosis. I don't know how likely that is, however.

I ended up going to a new plastic surgeon and I pretty much started over and I had a Latissimus dorsi flap surgery. He told me when he does fat grafting he puts lots of fat in. Most likely due to the reabsorption factor. I don't think my first plastic surgeon did. You might want to ask your surgeon about that. I think someone else mentioned that as well.
I didn't have fat necrosis but I think we're at risk of some type of necrosis with most of these surgeries.
By the way, you can't see either scar from the grafting- breast or belly! That was a plus.
Good luck with whatever you decide.

How important is a baseline MRI after mastectomy?

I had a Bilateral Mastectomy with no reconstruction and my oncologist says I don't need any tests done unless there are signs ? He does do blood work every 3 months on me ! I would prefer to have test's done so that if it came back some where they would catch it sooner so I worry about that ! Do what your Dr. thinks is best for you ! you could ask him why he wants the MRI and tell him your concerns ! good luck !

The only MRI I had was at time of Dx...no scans since bilat....my onc only does scans if patient is symptomatic.....I don't see my BS or PS anymore (my doing)...See my onc every 6 months (up to 5 years he saw me every 3 months)...labs and tumor markers prior to each visit...

If you become symptomatic it's too late. A yearly mri is a small price to pay to find any reoccurring problem early when it will be easier to treat. It's your call. I'm in to fight this with all I have. Wish you the best and hope your life is healthy long and full of beauty
