Yeah, you've hit the big two which work no matter what course of treatment you choose. You might consider making a list of potential questions (physically write it down). Don't be shy in asking anything no matter how ridiculous it might seem up front, then edit it down to what you really feel is important before you go in.

1st 3 month check up with Oncologist- any question ideas?

Ask when you will be getting the follow up scans. Usually you get them before the appointment and go over the results at that time. Since this is your first check up, your oncologist may want to see you before and after. As her/him how long the scans will take, and what you must do to prepare for them. For example a bone scan takes 45 min but the whole procedure takes about 3 hours because you get inject with a small amount of radioactive martial and need to wait 1-2 hours before the scan take place. You may have to be fasting for some tests and scans,
Find out what your scan/test schedule will be. After my nepherctomy I was scanned every 3 months for 2 years, then every 6 months for 3 years, after that once a year.

While typically I don't shut up, my mind seems to go blank some doc appointments!! I am comfortable and confident enough with my medical team that I can call and ask questions at any time. That being said, your questions may be different than another persons. Write down a list of questions that make sense for you.
